Nexus PCN is a collection of five practices serving a population of 40,000+ patients in the Exeter area. Combining the expertise and skills of Mount Pleasant Health Centre, South Lawn Medical Practice, Hill Barton Surgery, The Heavitree Practice & ISCA Medical Centre. All five practices now offer our patients easier access to multiple services which will benefit their long-term health.


By connecting primary care and utilising the new roles that coming together as PCN offers, we have extra clinical expertise which helps patients get the right care they need sooner. We also now have a stronger voice, allowing us to push for continuous improvement to the health and social care system while allowing us to easily integrate with the wider healthcare system and other local sectors including Community Services and Secondary Care.


How does this benefit me as a patient?


Nexus PCN is committed to the NHS Long Term Plan which is set out to improve the quality of patient care and health outcomes.

If you are a registered patient at one of our five practices, you will receive


· easier access and input from important services such as clinical pharmacy and mental health & wellbeing


· more time will be spent on improving the outcomes of those with health inequalities, for example, major diseases including Cancer, heart disease, stroke, respiratory disease, and dementia


· new and improved specialist services such as first contact physiotherapists meaning you don’t need to see and be referred by a GP first to get the care you need


· combined expertise of new roles working across all five GP practices


The Nexus team is led by existing leaders and GPs across our practices and includes a team of specialist roles covering, Clinical Pharmacy, Mental Health & Wellbeing, Occupational Therapy, Dietetics and more. Every role within Nexus PCN works collaboratively to improve the health and long-term care for all our patients.

How can I access Nexus services?


· To access the services of our PCN you will need to do so through your GP practice. Depending on the service you require, a healthcare navigator may be able to refer you, however, some services may require a doctor to make a referral first.
